DOG TRAINING:

I hereby acknowledge that I have voluntarily applied to participate in dog training activities with On Command Professional Dog Training. I am aware that there are inherent risks and hazards involved with and around dogs, and I am voluntarily participating in these activities with knowledge of these potential dangers. I am not relying on their trainers, instructors and assistants, hereinafter referred to as On Command Professional Dog Training to prevent such occurrences. In order to participate in On Command classes, demonstrations, or other activities, I being fully informed of such risks and hazards, agree to assume all risk of such occurrences. I hereby waive any and all claims or actions I or my guardians, representatives or assigns may have against On Command Professional Dog Training, and agree to release On Command Professional Dog Training from any liability in any and all personal injuries to myself, my dog, children in my charge, or harm to property caused directly or indirectly by any acts that might occur in conjunction with On Command Professional Dog Training or other activities. I also agree to assume sole responsibility for injury or damage caused by myself, children in my charge, or by the dog I own or handle during participating in On Command Professional Dog Training's events and activities, and further indemnify, defend and hold On Command Professional Dog Training harmless from any damage, loss, liability or expense, including costs and attorney fees, which results from damage caused by myself, children in my charge, or the dog I own or handle. I HAVE CAREFULLY READ THIS RELEASE OF LIABILITY AND FULLY UNDERSTAND THE CONTENTS THEREOF. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND CONTRACT BETWEEN ME AND ON COMMAND PROFESSIONAL DOG TRAINING, AND SIGN IT ELECTRONICALLY OF MY OWN FREEL WILL. Your completion of this form with your name in the "Dog Owner/Handler" field is the equivalent of your signature.

DOG BOARDING/GROOMING/DAYCARE:

I understand and agree that the employees, officers, directors and agents of On Command will exercise due care and diligence in the care and safety of my dog while in their care. In the event that my dog becomes injured or ill, I hereby give permission to On Command to seek whatever medical attention is necessary to treat my dog and I agree to pay all charges incurred in the treatment of my dog. I hereby consent to the veterinarian of On Command's choice to be retained for the treatment of my dog.

I understand and agree that while under the care of On Command, my dog will be interacting with other dogs and people and understand that there can be a chance of injury while interacting with other dogs and people and understand On Command will take all necessary steps to ensure the safety and well being of my dog in its care and I assume all risk of injury or illness to my dog while at On Command or while being transported to any veterinarian for treatment of such injury or illness.

I hereby release On Command, its employees, officers, directors and agents from any and all liability of any nature for injury, illness or death of my dog resulting from On Command's actions or the actions of my dog or any other dog while in the care of On Command or on the grounds of On Command except for actions resulting in negligence by On Command.

In the event that my dog causes injury or death to another dog or to a person while in the care of or on the grounds of On Command, I indemnify and hold harmless On Command from any action which may be brought against it, and for any defense, settlement or judgment entered against it resulting from such action of my dog. I also agree that if my dog causes injury to another dog or to a person while in the care of and on the grounds of On Command, I agree to pay the veterinary and/or medical expenses of the injured dog and/or person. I will assume all liability for the actions of my dog.

No pet will be released until all charges are paid in full or other arrangements satisfactory to our kennel have been made. The customer agrees to notify us in advance if there is any change in the pet's pick up date. Any pet left uncalled for seven days from the scheduled date of pickup, as noted herein, shall become the property of On Command. The owner of this pet agrees to pay reasonable legal fees and costs incurred by the kennel in the collection of outstanding bills.
